Mystery ring and mystery gem

I never find the mystery gem or ring at the equip shop nor as a reward from battles. Is this an error or should I look for it somewhere else?

Also, how many levels an equip needs to reach in order to evolve it from 4 stars to 5 stars?

You will find it at 10-10 normal mode and nightmare mode
