I can’t figure out why some stages I’ll only get 2 stars when all of my heroes are in great shape when I complete it whereas other times I barely squeak by or have a hero die and I’ll get 3 stars. I’m so confused.

Different stages have different star requirements. If you click on the i by the stage number, you should see it.
